Schedule: Full time

The Social Worker for Joseph’s Home will provide direct clinical care; program and care coordination activities and provide work supervision and oversight to peer/resident support associate staff; and will act as a field instructor and clinical supervisor for graduate and undergraduate students of social work.

Reviews admission applications, completes psychosocial needs assessments, develops individualized care plans, facilitates evidence-based and psycho-educational groups, and identifies high-risk residents and provides assistive services. In addition, the position will provide program and care coordination responsibilities, which include family conferences, coordinating discharges and transitions in levels of care. Coordinates or provides transportation to and from medical appointments and support services.

Bachelor’s Degree in Social Work or Counseling from a school of social work accredited by the Council on Social Work Education or Council for Accreditation of Counseling & Related Educational Programs. Master’s Degree preferred. Must have a minimum of 3 years’ experience in the human services area. Experience with homeless individuals preferred. Must possess a valid driver’s license.

Schedule: Part Time

The Peer Recovery Supporter (PRS) will facilitate one-on-one peer coaching, teach residents skills to navigate the health care system, facilitate group sessions focused on education and peer support intervention; transport residents/alumni to medical appointments, as needed; identify and communicate basic emotional or behavioral changes which require intervention or referral to clinical staff. Assist residents with accessing and developing natural support systems in their community.

High School graduate or equivalent required; college diploma in a related field is preferred. The Peer Recovery Supporter should be well-established in their own recovery. Must be certified through the State of Ohio as a Peer Recovery Supporter within a year of being employed at Joseph’s Home if not already certified upon hire. Must maintain continuing education requirements. A minimum of 2 years’ experience as either a Peer Recovery Supporter or in a role commensurate with a Peer Support is preferred. Must maintain a valid driver’s license.
