It Is Our Mission
Joseph’s Home, a ministry of the Sisters of Charity Health System, provides a nurturing, caring environment for men and women without resources who have acute medical needs, helping them heal and achieve independence.
Joseph’s Home is the only homeless service provider in Northeast Ohio exclusively focused on medical respite care. While men and women stabilize their physical illness in a nurturing faith-based environment, they also develop their individual housing plans and connections to behavioral health supports. Joseph’s Home now has two residence facilities: Joseph’s Home an eleven-bed facility with private rooms for men, and Mary’s Home, a ten-bed facility with private rooms for women. Services include ongoing medical supervision, medication management, nutritious home-cooked meals and coordination with local health care providers. Residents also receive intensive case management that includes development of permanent housing plans, benefit(s) review, transportation to and from any needed medical or housing appointments, and supportive programming that includes identifying community resources and supports.
The medical respite model fills a service gap between hospital and shelter. Medical respite programs serve people experiencing homelessness who are ready to be discharged from a hospital but too frail to recover on the streets or in shelters. The goal of respite care is to stabilize homeless patients’ medical conditions so they can transition safely to self care. With this model, Joseph’s Home residents qualify for additional housing options, including permanent supportive housing—an important solution for the chronically homeless who need housing with additional support systems.
Once residents leave Joseph’s Home, they can stay connected and continue to receive support through a robust alumni program that includes hundreds of graduates from over the years. Joseph’s Home encourages alumni to stay connected through intensive follow-up via in-home visits, telephone calls, mail and other forms of continuous contact. The Joseph’s Home Alumni Advisory Council includes former residents and is designed to encourage and coach current residents on their journey to improved health and wholeness. The Alumni Advisory Council also provides input into Joseph’s Home policy and protocols, and works to recruit additional alumni members and support other alumni and staff members to help achieve goals they set for each resident. Constant connection with alumni ensures effective long-term impact.
